How to use this tool

1
Choose the age group first

Child multipliers differ from adult ones.

2
Score each region’s area

Estimate the percentage of that region affected and pick the matching area score.

3
Score the four signs per region

Erythema, swelling/papulation, scratch marks and thickening, each 0–3.

4
Read the total and the band

Track the total over time rather than reacting to a single day’s change.

Formula & worked example

For each region: region score = area score × (erythema + oedema/papulation + excoriation + lichenification) × multiplier. Area scores: 0 none, 1 = 1–9%, 2 = 10–29%, 3 = 30–49%, 4 = 50–69%, 5 = 70–89%, 6 = 90–100%. Each sign scores 0–3, so severity sums to 0–12. Adult multipliers: head 0.1, upper limbs 0.2, trunk 0.3, lower limbs 0.4; children under 8: 0.2, 0.2, 0.3, 0.3. Total 0–72. Bands: 0 clear, 0.1–1.0 almost clear, 1.1–7.0 mild, 7.1–21.0 moderate, 21.1–50.0 severe, 50.1–72.0 very severe.

An adult with moderate signs (total severity 6) over 10–29% of the trunk (area 2) contributes 6 × 2 × 0.3 = 3.6 from the trunk alone; with similar trunk-only disease the total is 3.6, mild.

Frequently asked questions

The Eczema Area and Severity Index, the standard research and clinical score for atopic dermatitis severity.

Clear, almost clear, mild, moderate, severe and very severe, from the total score ranges.

You can track it, but scoring signs accurately takes practice; clinicians’ scores are the reference for treatment decisions.

A young child’s head is proportionally larger, so it carries more weight in the total.

In trials, a 50% or 75% fall (EASI-50, EASI-75) is commonly used as a meaningful improvement.
